The explosion in the number of cosmetic surgery procedures done in the U.S. has created a new demand – revision of unsatisfactory results, either because of unrealistic expectations or due to errors committed by the doctor.

Dr. Peter B. Fodor reports that revision work of procedures done elsewhere now account for fully half of his practice. Dr. Fodor is a board certified plastic surgeon and is a past president of the American Society for Aesthetic Plastic Surgery. He has long been an advocate for patient safety. Dr. Fodor says that correcting a previous procedure can be several times more difficult for the surgeon than a primary case, and so cosmetic surgery revision has become a sub-specialty of sorts.

No face-lift can stop time in its tracks, so as aging continues, even a happy patient may choose to have another one years later. But what if your face-lift or tummy tuck never pleased you? Maybe you ended up with more scarring than you bargained for, or your face lift left you with uneven features. Maybe just the general way things turned out aren’t what you had hoped for. Perhaps your first surgeon’s procedure gave you not only a tighter jaw line, but also a flat, wind-swept cheek and a stretched mouth. Or your nose no longer has an unsightly bump, but now is uneven, or worse, unnatural-looking.

The tremendous increase in the number of cosmetic surgery procedures over the past decade or so has created a new phenomenon – thousands of people who have been left unhappy with their results. These days, there’s such a critical mass of plastic surgery patients dissatisfied with their results that some surgeons perform secondary surgeries, or re-dos on a regular basis.

Nose jobs, for instance, are tricky because surgeons can’t control healing or how good the building materials are. Cartilage can be too thick or too flimsy; skin draped over a newly fashioned nose structure might not shrink to the shape the surgeon wants.

Dr. Fodor says that sometimes advertising can create unrealistic expectations. The idea that a procedure can be quick or painless is often marketed to patients. “The patient is always right when it comes to plastic surgery”, says Dr. Fodor. “A heart patient usually doesn’t decide if she wishes to spend less money to have a less-effective procedure. But a cosmetic surgery patient can choose a minimally invasive face-lift instead of a more complete one. If one surgeon won’t give her what she is asking for, another one will, not always ending with the best result.”

In many of the so-called “mini” –face lifts, problems are often under corrected, according to Dr. Fodor. These short-cuts may require little recovery but usually don’t last very long.

Minimally invasive! Non-Invasive! These words have become a mainstay of marketing and news coverage of cosmetic procedures offered by many physicians and med spas. It seems that every week there is an announcement touting a new “miracle” laser to rejuvenate the face, or a new filler material for plumping and obscuring facial lines and wrinkles. But what are people really buying when they choose one of these treatments, in hope of gaining a younger look?

“Primarily, they are buying a little time,” says Dr. Fodor, who has participated in the evaluation of many of the cosmetic lasers and injectable fillers that have been released over the past twenty years. “For some people who have minor issues including fine lines, sun damage and minor skin laxity, laser treatments and products like BOTOX and Juvederm can produce results,” say Dr. Fodor

Many people don’t realize that the minimally invasive procedures offer only temporary improvements and they must be repeated (every few months, in most cases) which will increase by many times the long-term cost of maintaining one’s appearance. Dr. Fodor says that the right plastic surgery procedures can provide much more dramatic aesthetic improvements, at a lower overall cost because patients won’t need to return repeatedly for re-treatments.

to the rest of the body. The Baby Boomer Generation is entering the age of a variety of hormonal changes. These changes result in a tendency to develop undesirable fatty accumulations over the abdomen (waist “muffin top”), under the waist ("love handles,” “watermelon rolls,” and "hip rolls") and under the buttocks at the top of the thighs ("banana rolls") as well as on the inner thighs and inner knees. Women who have always experienced difficulty with "saddle bags" can also notice an increase in the outer thigh area. In addition to the abdomen and hips, men commonly notice an increase in volume of the breasts.
Many of the most enthusiastic exercisers notice that regardless of which form of exercise and which method of dieting they subscribe to these fatty accumulations seem to change very little. Frustration over this lack of results can lead to a loss of will to continue. Subsequently, more weight is gained in this and other parts of the body, leading to more frustration, obesity and depression. And for some, a yo-yo dieting habit can develop.
Dr. Fodor believes that the answer to this vicious cycle that occurs with the desire to achieve permanent fat and weight loss rests with a combination of liposuction, properly selected exercise programs, and an appropriately designed food intake. Strategic suctioning around the buttocks will create a shapelier and/or more compact looking, lifted backside. For some patients, Dr. Fodor may also recommend a tummy tuck, either at the same time or in different surgical sessions...
Initially, when liposuction was first developed, only the deep layer of fat was suctioned. This approach, respecting the superficial layer of fat, serves as a cushion under the skin, camouflaging potential waves, depressions, and irregularities. It works well for patients with elastic skin and for smaller fat deposits.
A more recent innovation in liposuction pioneered by a few surgeons including Dr. Fodor; is known as Superficial Liposuction and is currently being practiced by only a handful of the most experienced and highly skilled plastic surgeons along with Dr. Fodor. Superficial Liposuction works closer to the undersurface of the skin. It is far more difficult to perform evenly, but when properly done a better skin contraction result will occur and, therefore, this type of procedure can be used to treat older patients and those patients who have larger accumulations of fat.
The array of options available to patients today, including lower and upper layer liposuction, make it possible for liposculpture to create the visual impression of a body lift. In conjunction with liposuction, some patients also receive breast lift surgery or breast reduction.
Liposuction, also known as “Lipoplasty” and “Suction-Assisted Lipectomy,” should only be handled and performed by a highly trained, board certified plastic surgeon with a proven safety record and a very low complication rate. Dr. Fodor’s patients’ discomfort following the liposuction procedure is similar to what most patients are used to experiencing after a heavy workout. Once more, the results are permanent as witnessed by hundreds of thousands of patients who have had liposuction. Fat cell re-growth extensively studied at the site of suction and elsewhere in the body, has been found insignificant. Because they no longer have body regions that resist efforts to diet and exercise, most patients can lose additional weight after suction more easily than before the surgery.
Over the last two decades, Dr. Fodor's personal contributions to refining all aspects of liposuction (from fluid infusion to techniques and instrumentation of extraction as well as postoperative care) have made this procedure extremely safe and, for most patients, painless. Due to Dr. Fodor's impressive technique, liposuction's results are highly predictable and patients now attaining significant aesthetic improvement. In this case, liposuction can be better defined as "liposculpture.” Continue reading →

The Baby Boomer Generation is entering the age of a variety of hormonal changes. These changes result in a tendency to develop undesirable fatty accumulations over the abdomen (waist “muffin top”), under the waist (“love handles,” “watermelon rolls,” and “hip rolls”) and under the buttocks at the top of the thighs (“banana rolls”) as well as on the inner thighs and inner knees. Women who have always experienced difficulty with “saddle bags” can also notice an increase in the outer thigh area.

The answer to this vicious cycle that occurs with the desire to achieve permanent fat and weight loss rests with a combination of liposuction, properly selected exercise programs, and an appropriately designed food intake. Strategic suctioning around the buttocks will create a shapelier and/or more compact looking, lifted backside. For some patients, a tummy tuck, either at the same time or in different surgical sessions may also be recommended.

A more recent innovation in liposuction pioneered by a few surgeons is known as Superficial Liposuction and is currently being practiced by only a handful of the most experienced and highly skilled plastic surgeons. Superficial Liposuction works closer to the undersurface of the skin. It is far more difficult to perform evenly, but when properly done a better skin contraction result will occur and, therefore, this type of procedure can be used to treat older patients and those patients who have larger accumulations of fat.

Many board-certified plastic surgeons routinely see patients in need of secondary or re-do operations. In my own practice, at least 50 percent of the patients consulting with me about liposuction are seeking corrective surgery. Many, but unfortunately not all, such patients can be helped and their results can be improved. However, it is important for these patients to have realistic expectations; an outstanding outcome from a re-do is less likely, even in experienced hands, than if the procedure were being performed for the first time.

When liposuction is performed without discretion, artistic sense, or technical skill, the results can be unattractive and very discouraging to patients. Most commonly, secondary liposuction is performed to treat asymmetry, surface irregularities, and/or contour problems caused by excessive fat removal.

Treatment of poor liposuction results often includes strategic suctioning to correct asymmetries and areas that were insufficiently reduced by the primary procedure. In other instances, areas over-reduced by the primary procedure are treated by autologous fat transfer. Very commonly, a combination of these two techniques is used to improve on less-than-satisfactory results obtained by another surgeon.

Both BOTOX® and Dysport® are derived from a specific form of bacteria. Most people do not realize that these injections were originally utilized as a remedy for neck and facial spasms. Then of course BOTOX® and Dysport® treatments began to be used more effectively for cosmetic purposes–they are an extremely efficient approach to softening or removing the natural wrinkles between the eyes (frown lines), on the forehead, at the sides of the eyes (“crow’s feet”), at the base of the nose, around the lips, etc.

Injected through a very small needle into the muscles of the face to stop nerve impulses from reaching the muscles causes these muscles to relax, these treatments which makes wrinkles gradually disappear. The results last three to six months and, at times, longer.

I have extensive experience using BOTOX® Cosmetic not only for cosmetic reasons, but to treat headaches (including migraines) and excessive sweating of the armpits and palms.

Injectable treatments are a wonderful tool in treating cosmetic patients but, like any tool, they can be misused. Inadequately trained practitioners from varied backgrounds are often responsible for the substandard results that all of us have seen, many times on well-known celebrities who appear virtually expressionless from Botox, sporting “over plumped” lips and “mask-like” faces from over zealous use of fillers. These kinds of results are entirely avoidable.

I practice meticulous technique with careful control of the location and depth of the injections, based on an intimate knowledge of the anatomy gained in no small measure from my experience in endoscopic facial rejuvenation.

As a patient, the best way you can minimize liposuction risks is to ensure that you have chosen a surgeon with appropriate surgical training, sufficient aesthetic sense, and technical knowledge specific to liposuction. In addition, you should learn as much as possible about all the details of the procedure. Here are some guidelines that will help you select the best surgeon to perform the procedure and minimize liposuction risks:

Look for a surgeon certified by the American Board of Plastic Surgery.

Get documentation of the surgeon’s privileges to perform liposuction at an accredited acute care hospital with proper support systems.

Be sure to give an accurate and up-to-date medical history to your surgeon. Report all past surgeries — including all past cosmetic procedures and medications you are taking, including dietary and herbal supplements.

Discuss all aspects of your liposuction procedure with your surgeon beforehand, including risks and proposed recovery time. Learn who will oversee your recovery.

Learn who will be administering anesthesia, and what type of anesthesia will be used.

View before and after photos to familiarize yourself with your surgeon’s work. Do the post-surgery photos reflect a result that is dramatic but complements the patient’s natural body shape? Do they show a smooth, graceful contour with no change in skin pigmentation?

Following these guidelines will help avoid liposuction complications and the need for a corrective, secondary liposuction procedure. Though it is impossible to completely eliminate the element of risk from any surgical procedure, knowing as much as possible about your surgeon and the procedure is one of the keys to a safe and effective liposuction experience.
